So my friends and I had a really strange issue on a private modded Minecraft server (online and fabric 1.21.11) and want to know if anyone has seen something similar.
The server has been running for over 200 in-game days. All mods were downloaded from Modrinth, and as far as we know they’re common/trusted ones, like Biomes O’ Plenty and a minimap mod. No one on the server has OP, and we checked the logs with the two server owners.
About a day ago, one of my friends was in the Nether when chat messages started appearing from something claiming to be the “server.” Around the same time, there were strange audio effects like whistling, footsteps, and a heartbeat. Nothing appeared in the subtitles.
We started interacting with it and messed around with it and it responded to what we said and could do things like:

Later it gave us coordinates, so we went there. When we arrived, it spawned more withers and then around 10 wardens in the ocean. We restarted the server after that.
When we logged back in, a name-tag appeared on my lantern saying “{Miss me?}” After that, it said things like “I’ve done too much” and “you’ll see my power later.” Since then, the activity mostly stopped, but the next morning part of our house was missing and random blocks had been deleted. It also seemed to know where our base was.
Today, one of the owners and I removed and reinstalled all the mods individually. Nothing unusual has happened since, but it had already gone quiet before that, so we’re still not sure whether the issue is gone. We also checked the startup and latest logs and didn’t find anything obvious.
Has anyone seen anything like this before? We’re trying to figure out what this is - I've researched a lot and nothing has shown up. Also, we do not have any plugins or modpacks that we are aware of.

- Teleport players (to world border, nether roof, end, our 'home')
- Give status effects like levitation and blindness
- Spawn mobs including withers and wardens
- Give and remove items
- Clear inventories
- Temporarily give creative mode
- Pretty much any OP command
I'll list a few examples of things that happened while we were 'toying' with it.
- One friend said he wasn’t afraid of the "server", and he got teleported about 10,000 blocks away and attacked by withers.
- I asked to fly and got levitation for about a minute.
- Someone asked to be teleported to the End, and it happened even though we hadn’t gone there yet.
- I asked for diamonds while on the Nether roof and got a full inventory, but they disappeared after about a minute.
