One player consistently reporting extremely high μs/t in Observable profile, struggling to diagnose the cause.

Hello! I've been running a modded 1.20.1 Forge server for roughly a month or so, and for most of that time (but especially lately) one player in particular has consistently been shown to have a very high μs/t cost in the Observable profiler (ranging from 6k - 28k) and at times causes the server to slow down. I was looking through the Spark report to see if I could maybe get a hint as to what exactly is causing it, but the best I got was tracing it down to here, and I'm not really sure how to tell what this is (or if I'm on the right track even).

We tried removing a number of things from his inventory/equipment as well as just completely emptying out his inventory to see if that would cause any change, but it didn't seem to have any effect.
